Initial consultations
Please bring the following items to your first consultation:
– A referral from your doctor, dentist or specialist (when applicable)
– A Medicare card
– The card you received from your health insurance provider (if applicable)
– A completed patient registration form can be downloaded here
– Scan or X-rays of the problem you are currently experiencing
Blood tests if necessary
As part of your first consultation with Dr. John Doe, he will ask you questions about your health status and history. He may perform a physical examination. He will examine your scans, X-rays, and blood tests. In some cases, additional blood tests or scans are needed.
Your options will then be discussed with Dr. John Doe. He will discuss the pros and cons of surgery with you in detail. In addition to explaining what to expect before and after surgery, Dr. John Doe will also provide the usual instructions.
In the event surgery is needed, it can be booked at this consultation.

Questions and concerns are likely to arise. Discussions about them are appropriate at this time.
Following your initial consultation
Following the consultation, you can book your surgery if it is needed. We will book any hospital appointments you need on your behalf via our administration staff.
In the beginning, you will need to pay a fee. You will be reimbursed for some of it by Medicare. If you would like, we can file your Medicare claim for you.
The cost of surgery
An initial consultation with Medicare will be partially reimbursed.
A quote for surgery will be provided to you by Dr. John Doe. In order to be sure of how much of the cost of treatment will be covered, we will help you submit private health insurance claims or Medicare claims (if applicable).
We will provide you with the anesthetist’s contact information so that you can ask for their fee schedule if an anesthetist is involved. The anesthetist’s cost is partially covered by Medicare and private health insurance.
It is mandatory for children under 18 to be escorted by an adult. In the event that you need a Spanish interpreter, please let us know when you book your appointment.
